I have scheduled my appointment. How can I prepare?
Once you schedule your appointment, we will e-mail you a new patient packet. We ask that you fill out these forms before your appointment or allow 15 minutes to fill out paperwork at our office, prior to your appointment.
What should I do if I have dental insurance?
Please provide us with the information prior to your appointment to allow us time to provide a benefits breakdown.
What if I need to cancel my appointment?
We reserve the right to charge for appointments canceled or broken without 24 hours advance notice.
